Guardian Authorization
For when you are traveling and your child is left at home with a caregiver or guardianThis is to be filled out and given to your child’s caregiver/guardian while you are away. Giving them authority to make quick decisions based on the information available to them. Please discuss concerns or important information with them before you depart for your trip.
Also attach the Emergency Form Below.
Minor Authorization
For when your Child is traveling with another person or group. This needs to be filled out for each child prior to travel. Both parents must sign along with the designated guardian. It is not always necessary, however, if your child is under the age of 17 it is a good rule of thumb to always provide this document.
